If you have insurance coverage, we will prepare and submit a standard insurance claim form to your insurance company for services rendered, however, it is the responsibility of the patient and/or legal guardian of the patient to make any payments not covered or denied by the insurance carrier. It is your obligation regardless of any other circumstances.
If you have any questions regarding a recent bill or insurance coverage, we have exceptional financial counselors and patient account representatives that would be happy to assist you.
Most patients are covered under a Managed Care Plan, however, all plans require different information and steps be taken in order to approve the services rendered. Our office staff will be able to tell you if our doctors are or are not on your plan and will provide you with a quote of benefits or coverage for the service rendered. Attached is a general list of insurance plans we accept.
Please know that insurance plans may vary by physician. If your plan is not listed in our Accepted Plans please contact your insurance company to verify that we are providers or call us at 972-250-5700 and we’d be happy to assist and verify if we are plan providers for you as our list is a general list for your convenience.
